The Application Process
Make the Initial Call
Call your Regional Center Intake Referral Line to begin the process.
Determine Eligibility
The intake coordinator will help determine if your child is eligible. You will be assigned a case worker.
Complete Applications
Work with the Regional Center case worker to complete all necessary paperwork and assessments for Regional Center services, SDP, Medi-Cal, and IHSS.
Develop Care Plan
Work with an Independent Facilitator to develop a personalized care plan that addresses your child's needs.
